Purpose. Drivers complete this checklist before the first trip of the day and before every departure with a load, to confirm the vehicle is roadworthy and the load is restrained and within mass limits.

1.When to complete

Complete the pre-start section before the vehicle moves at the start of the shift. Complete the pre-departure section before leaving the dock with each load. The checklist is completed on the driver device and a copy is attached to the fleet register.

2.Pre-start inspection

Walk around the vehicle and trailer checking tyres, wheel nuts, lights, indicators, mirrors, windscreen, wipers, horn, fluid levels, air pressure build-up, brake test, coupling and load restraint equipment. A defect that affects safety must be reported to the allocator and the vehicle must not be driven until cleared.

3.Pre-departure load check

Confirm the number of pallets against the manifest, confirm the declared mass is within the vehicle's rating, check that restraint meets the Load Restraint Guide for the load type, and check that dangerous goods placards and documents are in place when required.

4.Fitness for duty

By signing the checklist the driver declares that they are fit for duty, have had the required rest, and have completed their work diary for the previous period.

5.Defects and sign off

Record every defect, even minor ones, so that maintenance can be scheduled. The driver signs, and the allocator counters signs when a defect has been assessed.

6.Complete the form
